Small Business Utility Disruption Prevention Program Pre-Application

PRE-APPLICATION CHECKLIST

Applicants are required to complete the pre-application screening below. Upon successful completion of this screening checklist, the applicant will be provided with the link to the full application.

Please note that a submittal of an application does not guarantee an award of financial assistance. Funds will not be disbursed for approved applications until your utility provider has provided the proper verification.

  1. My business has experienced a loss of income that occurred after March 17, 2020 due to the COVID-19 public health emergency that has caused significant hardship in paying our electric or natural gas utility bills.

  2. My business is eligible to apply for assistance (ineligible businesses include: Adult Entertainment; Construction, Internet Sales, without corresponding storefront; Medical; Private Clubs, Professional Services; Professional Sports; Religious Institutions, with the exception of those offering social services including daycare, food bank, preschool, shelter, etc.).

  3. My business had 50 or fewer employees as of March 1, 2020. (For this program, an employee is an employee without regard to the number of hours worked - one person = one employee). Applicant to complete a payroll template with employee information.

  4. My business operates from a commercial location in Iowa. (May not include businesses located in a residential location or those who exclusively offer internet sales.)

  5. I'm aware that if my application is approved for program support that a single, one-time payment will be issued directly to my utility provider who will deposit those funds in my respective account as a credit which will be applied to my outstanding account balance.

  6. My business is in good standing with business or professional licensing entities (including the Alcoholic Beverages Division), if applicable. I understand that I will be required to provide my license number to allow IEDA to confirm good standing.

  7. To determine if my businesses’ average monthly electric usage is less than 25,000 kWh and whether my monthly average natural gas usage is less than 2,500 therms, I understand that I need to review a copy of my utility bills and/or contact my electric and natural gas utility providers to confirm.

  8. If I own multiple businesses or locations, I will complete separate applications for each distinct business/location that is eligible for assistance.

  9. I have a completed payroll template indicating employee information as of March 1, 2020 in an electronic format and ready for upload to the online application. Download a sample template form here. [.XLS]
  10. I certify that I did NOT receive an IEDA-issued grant from the Small Business Relief Grant program.

  11. I certify that I did NOT receive an IEDA-issued grant from the Nonprofit Recovery Fund.

  12. I understand that providing false, misleading, or incomplete information may result in the application being declared void. I understand that it is a criminal violation under Iowa law to make a false statement in writing for the purpose of procuring assistance from a state agency.

  13. I understand that in order to be considered for assistance, IEDA must contact my utility. I authorize the Iowa Economic Development Authority to contact my utility provider(s) regarding the account number(s) for which I am applying for assistance. Additionally, I authorize my utility provider(s) to release the requested information to IEDA.

  14. I understand that an application submission is a not a guarantee of financial assistance.
